Job Description

Evaluate and treat patients of all ages, adapting the care plan when necessary. Serve as a clinical mentor and resource for staff and students.

• Perform patient transfers with appropriate assistance, using safe body mechanics to minimize the risk of injury.
• Show respect and empathy towards patients and colleagues, and provide fully engaged care.
• Develop and adjust individualized care plans based on patient response and evidence-based practices.
• Educate patients, family, caregivers, and other healthcare providers effectively.
• Evaluate and diagnose physical therapy needs based on clinical findings.
• Document care in compliance with Clinical Process Policy (CTSCP67).
• Screen patients to determine the need for therapy services.
• Supervise support personnel and provide regular feedback.
• Evaluate orthopedic and neurologic impairments, and create functional deficit-based care plans.
• Conduct objective testing and therapeutic modalities as appropriate.
• Communicate patient progress and treatment recommendations with referring providers.
