As our Assisted Living Transition Specialist, you will play a vital role in supporting the Assisted Living Residency Counselor here at Beatitudes Campus. Your primary responsibility will be to assist seniors and their families, as well as existing Independent Living Residents, in transitioning smoothly into our assisted living or early memory support residences. You will provide guidance, support, and personalized care to ensure a seamless move-in process. Your excellent communication skills, empathy, and attention to detail will be crucial in helping each resident and family member feel comfortable, and confident in their new home.

  • Along with the Assisted Living Residency counselor, the Transition Specialist is responsible for:
  • Scheduling and assisting with assisted living/memory support move-in activities
  • Assisting with inquiry calls, gather information, schedule tour for assisted living residency counselor
  • Maintaining room turn report, ensure rooms are being turned in timely manner / on time for new move in
  • Participating in in success matters team weekly
  • Conduct daily walk throughs of both neighborhoods, notating anything needing repair/housekeeping
  • Attending interdisciplinary meetings as appropriate (e.g. stand-up meetings, care-plan meeting, pre-move-in meetings, etc.)
  • Maintains turnover sheet that keeps track of ins and outs of residents via daily census report
  • Walks and inspects apartments prior to final inspection by the Assisted Living Residency Counselor; and, before resident move in. Must ensure room is ready and all necessary requests have been completed.
  • Schedule housekeeping for final move in touch up.
  • Schedule phone, internet and tv set up PRIOR to move in
  • Maintain move in packets/marketing material.
  • Help current resident transitions from independent living to assisted living/early memory support; and, assisted living to early memory support.
  • Prepare move in paperwork for signature.
  • Maintain and distribute new resident handbooks.
  • Maintain inventory of ALTCS provided furniture, order as needed.
  • Maintain list of ALTCS approved provided room necessities (phones, etc.)
  • Maintain respite rooms – readiness, phone connections, tv hookups, etc.

  • Bachelor's Degree preferred.
  • Proficient with Microsoft Word, Excel and Outlook.
  • 2 years Health Care Sales experience or equivalent industry experience preferred.
  • Experience with Arizona Long-Term Care System is a plus.

Compensation details: 20-20 Hourly Wage
